On Monday, April 1st, PentaHealth practices will switch to a new record system: EPIC. EPIC is the most widely used records system in the US, and is used by major health systems, including Temple University, Penn Medicine, Main Line Health, and Jefferson Health. While we are working to make this transition go smoothly, we expect some service interruptions during this time. We are asking for patience with us as we adopt this new system.
- Starting 4/1 we will have a new appointment confirmation system and Pre-Check-in process.
- Appointment confirmations will be sent out via text 3 weeks, 3 days, and 22 hours prior to your appointment.
- Pre-Check-in will be sent with your appointment confirmation 3 days prior to your appointment. Please complete your pre-check-in to save time.
Pre-Check-in is a feature that allows patients to verify or request updates to their information such as demographics, (address, preferred language, home number, cell number, work number, preferred number, & e-mail address), preferred pharmacies, medications, allergies, and health issues. You can complete questionnaires (where applicable) and even sign certain consents electronically, up to 3 days prior to your appointment.
We are also thrilled to announce that as of April 1, 2024, PentaHealth patients will have access to MyChart, an electronic medical record by Epic. This will be an innovative patient portal designed to enhance your healthcare experience.
During this transition period, we also expect a higher volume of phone calls. If you have an active MyChart by Penn Medicine, you can send us a portal message. We will be prioritizing phone messages that need urgent responses. While we usually commit to responding to messages timely, we expect that we will need more time during the first two weeks of April.
Although MyChart will display MyChart by myPennMedicine, we very much remain an independent medical group separate from Penn. MyChart will replace PentaHealth’s current patient portal, FollowMyHealth. Your PentaHealth clinicians, how you schedule appointments, and where you receive care will not change.

MyChart enhances the features that PentaHealth offers in its current patient portal to empower you in your healthcare journey.
Here’s just some of what you can do with MyChart:
- Securely access your health information, including appointment details, medications, and test results, from anywhere, at any time.
- View upcoming tests, procedures, and appointments to help you stay on track and make sure you never miss a visit.
- Easily communicate with your healthcare providers and receive responses for non-urgent messages.
- Keep track of your personal medical and family histories, which helps your doctors understand your health and better prevent or treat certain conditions.
Sign up for MyChart:
If you don’t already have an account, you will get an activation invitation the next time an appointment is created, or you arrive for your next visit. The activation code will come via text message or email.
Follow the instructions to create your MyChart account. Sign-up instructions and an activation code will also appear on your After Visit Summary or billing statement.
If you already have an account, you can login to MyChart by myPennMedicine at any time. If you haven’t visited the portal in a while and forget your login details, you can recover your username or reset your password.
What will happen to FollowMyHealth?
Wednesday, March 27:
Messaging through FollowMyHealth will be turned off at 5 pm to allow time to transition to our new Electronic Medical record. Please be careful sending prescription refills and medical questions during the last week in March. We encourage you to make any requests to the office prior to 3/27 if using the portal. This includes refill requests that might be due in late March or early April. PentaHealth will hopefully get to every question and refill during that time but if you feel like you had not gotten a timely response to one of your requests, please call our office for assistance.
FollowMyHealth will be available in read-only mode starting on April 1. You will be able to view your past information, but no updates will be made in that portal. Prescription refill requests, messaging with your care team, and other important functions must be done in MyChart.
