We employ health coaches and a team of 15+ dedicated nurse care managers that provide longitudinal and episodic care management to patients that need additional attention. We strive to see patients as quickly as possible after a hospital discharge and have a team of nurse practitioners that will travel to the patient’s home when necessary.
