We are a leader in value-based care and there are significant pay for performance and gain sharing opportunities for physicians. We are currently participating in Medicare’s Shared Savings Program (MSSP), Primary Care First, and many commercial and Medicare Advantage quality programs. We are electronically connected to all local hospitals and laboratories and are well equipped to handle population health. Our quality numbers are high and our readmission rates are low.

We employ health coaches and a team of 15+ dedicated nurse care managers that provide longitudinal and episodic care management to patients that need additional attention. We strive to see patients as quickly as possible after a hospital discharge and have a team of nurse practitioners that will travel to the patient’s home when necessary. We are dedicated to providing high-quality care, and rewarding physicians for the work they do.
