We are a leader in value-based care and there is significant pay for performance and gain sharing opportunities for physicians. We are currently participating in Medicare’s Shared Savings Program (MSSP) and many commercial and Medicare Advantage quality programs. We are electronically connected to all local hospitals and laboratories and are well equipped to handle population health. Our quality numbers are high, and our readmission rates are low.

We employ several experienced Social Workers and a team of 10+ dedicated nurse care managers that provide longitudinal and episodic care management to patients that need additional attention. We do transitional care management outreach with all patients who are discharged from an ER, a hospital, or a skilled nursing facility and we pride ourselves in seeing patients as quickly as possible after these visits or admissions. We are dedicated to providing high-quality care and rewarding physicians for the work they do.
